Student Health and Immunization Forms
All students must provide the School with a general health evaluation and record of
immunizations. The health form and proof of required immunizations forms are for this
purpose. To access and print copies of these forms, click on the link below. You should
complete pages 1-3 and have a physician complete pages 4-5. Return the form at the
time of registration, not before. Your health information will be treated
confidentially. Health evaluation forms and immunization records are maintained by the
Student Health Office and are not made a part of the student’s education record.
