IMPORTANT: Florida Residency Documents are due Friday, June 19th, 2009
If you’ve been admitted as a resident of Florida you must decide whether you are dependent or independent and provide all of the documents listed below. If you did not claim Florida residency when you applied to the Miller School of Medicine you can ignore this section.
Being certified as a Florida resident is a simple process if you or your parents have developed “significant” ties to the State of Florida and they or you have lived in the state at least since last August (12 months prior to the first day of class)—and kept records. To help you decide whether you are a dependent or independent student please refer to the page of different scenarios that address most of the situations in which our incoming students find themselves.
- Complete a residency statement (print it, fill it out, and send it to us) and
- Provide the appropriate set of documents listed below. DO NOT mix documents from one list with documents from the other list (e.g., don’t send your parent’s income tax information with your driver’s license, etc.). Mail the documents to the Office of Admissions or FAX them to 305-243-6548 (secure FAX machine in the Admissions Office).

Documents required for students who are DEPENDENT
1. 2008 Federal Income Tax Return
2. Property tax notice/homestead exemption receipt
3. Driver’s license, voter card, auto registration
4. Professional/business licenses
5. High School Diploma
6. College/University Transcripts and fee statements
Provide transcripts from all colleges and universities that you attended. If you attended a college or university in Florida (even if just for the summer), provide evidence that you were a Florida resident “for fee paying purposes” (latest fee statement, tuition bill, etc.).
1. 2008 Federal Income Tax Return
| The tax return required is that of the individual(s) who has (have) maintained residency in Florida for at least one year prior to August, 2009, and who claimed you as a dependent in 2008. Only the first page and the signature page are required. The first page must show the address in Florida from which the return was filed and show you listed as a dependent with your social security number and relationship to the person claiming you. The second page must show the signature of the person claiming you as a dependent or the stamp of the professional preparer. |
2. Property tax notice/homestead exemption receipt
| Provide the latest (2008/2009) homestead exemption certificate for the place of residence of the person(s) residing in Florida who claim you as a dependent on their Federal income tax return. Property tax notices may be sent if they show that the property qualified for homestead exemption. If the person who claims you as a dependent rents instead of owns the residence, provide rent receipts or rent agreement for the period of August, 2008, through August, 2009; and any other evidence that this has been their primary residence in Florida for at least 12 consecutive months prior to August, 2009. |
3. Driver’s license, voter card, auto registration
| For the person(s) who reside in Florida and claim you as a dependent, provide copies of their Florida driver’s license (include the back if it has been renewed), their voter registration card, and latest auto registration. All documents must be current. The driver’s licenses and voter cards must have been issued before August 2008. |
4. Professional/business licenses
| Provide a copy of all professional and/or business licenses issued by the State of Florida to the individual(s) who claim you as a dependent. |
5. High School Diploma
| If you graduated from a high school in the State of Florida you must provide a copy of your diploma or your permanent high school record. |
6. College/University Transcripts and fee statements
Provide transcripts from all colleges and universities that you attended. If you attended a college or university in Florida (even if just for the summer), provide evidence that you were a Florida resident “for fee paying purposes” (latest fee statement, tuition bill, etc.).

| Provide a copy of your 2008 Federal Income Tax Return. Only the first page and the signature page are required. The form must show the address in Florida from which the return was filed. Your signature or the name of the professional preparer must be clearly visible. Also provide copies of W-2 forms for the wages you earned while working in Florida for the 2008 tax year and an employment verification letter with the dates of your employment. |
2. Property Tax Notice/Homestead exemption/rent receipts
| Provide a copy of your latest property tax notice and/or homestead exemption certificate for the home in Florida that you own. If you rent, provide copies of the lease and/or rent receipts covering the period from August, 2008, to August, 2009. Provide any other evidence that you have to show that you maintained this domicile for at least 12 consecutive months preceding the start of medical school. |
3. Driver’s License, voter card, auto registration
| Provide a copy of your driver’s license (and the back if it has been renewed), your voter registration card, and auto registration in your name. These documents must be current. Your driver’s license and voter card must have been issued at least one year prior to August 2009. You can also submit a “declaration of domicile” filed at a local Florida courthouse but do not send utility bills, bank statements, or insurance cards. |
4. Professional/Business Licenses
| Provide a copy of all professional and/or business licenses issued to you by the state of Florida to practice a profession or operate a business. |
5. High School Diploma
| If you graduated from a high school in Florida you must provide a copy of your diploma or your permanent high school record. |
6. University/College Transcripts/Fee Statements
| Provide transcripts from all colleges and universities that you have attended. If you attended a college or university in Florida, provide evidence that you were declared a Florida resident for fee paying purposes (fee schedules, tuition bills, etc.). |
