The Registrar's Office requires a number of forms to be printed, filled out and brought to Orientation.
The forms are due at the Registration portion of Orientation and are required before you can start classes.

The forms required by the Registrar's Office--and any associated instructions--are detailed below:
  1. Please download the Code of Honorable and Professional Conduct form, print out the form, fill it out as per the form's instructions. Please bring this completed form to the Registration portion of Orientation.

  2. PPD testing will be done at registration in the student lounge (2nd floor), in the Rosenstiel Medical Science Building at the UM Miami Medical Campus. If you have already done a PPD, please bring a copy for the Student Health Office, they will be collecting this information at registration for your medical records file.
    For students at the regional campus, PPD testing will be done at registration in the student lounge in the Charles E. Schmidt College of Biomedical Science on the regional campus at FAU. If you have already had a PPD, please bring a copy for the Student Health office, they will be collecting this information at registration for your medical records file.

  3. Please sign in to myUM (see our page about your University Account) and update your local address and phone number. Follow the online instructions for logging into the CaneID Authentication System which will then take you to the myUM system.

  4. Appropriate attire for orientation is "professional casual" (no t-shirts, shorts or flip-flops).

  5. Please be sure to fill out the Health and Immunization Form. This is due no later than July 31 (before registration).

  6. Please make sure you do your OSHA Training.

Be advised that students are required to complete HIPAA requirements every year, please do the following:
  1. Go to the Privacy / Data Protection Project homepage.

  2. Read the following modules:

  3. Complete the following quizzes:

  4. Please download the HIPAA Certification Form. Please bring this completed form to the Registration portion of Orientation.
